Financial Information

Here at Eastern Virginia OMS, our fees are based on the type and the extent of treatment you require. If you provide us with your current medical and dental insurance information and cards, we will file your claims for you. Surgeries will be pre-authorized and estimates of co-payments, deductible, and balances due will be advised. All financial arrangements will be openly discussed during the time of your consultation visit.

Please advise us of any changes in your address, health status, phone number, or insurance.

• Both of our offices participate with most MAJOR insurances. However, it is advised that you contact our office for a current status of our insurance participation.
• Please note that any deductibles, co-pays, non-covered services, and balances that are due are the responsibility of the patient at the time of his or her service.
• We ask that you do not hesitate to contact our practice administrator with any additional questions or concerns that you may have.
